HOW OFTEN WILL A HOME HEALTH CARE PROFESSIONAL VISIT MY HOME?
Your physician, in conjunction with our Registered Nurses, will develop a plan of care specifically designed to meet your health care needs. The frequency of visits to your home can be varied according to the degree of your iIlness. Our goal in home care is to provide a viable alternative to institutional care. Our professionals are available in your home for treatment, if needed, on a daily basis and if ordered by your doctor. At the time of admission, we will be glad to discuss your individualized plan of care.
GUIDELINES SPECIFIC TO MEDICARE
In order to receive services under Medicare, the patient must be homebound. A homebound patient is defined as one who requires considerable effort and assistance to leave home.
